Reminder:
One complete physical per year is required by the Pennsylvania
Interscholastic Athletic Association to participate in high
school athletics. The physical examination can be completed
by your family physician or by Allegheny General Hospital Sports
Medicine at a cost of $ 10.00 (cash or checks made payable to
AGH) on the above date. Attached is a medical history form that
must be completed and brought to the physicals. Physicals will
last approximately 45 minutes. The athletes should wear T-shirts
and shorts the day of the physicals. If you have any questions,

Additional
Reminder: In order to have
a physical examination completed, the additional HIPPA release
form must be signed by a parent, guardian, or athlete (if
they are 18 years old). |
